Central hyperthyroidism combined with Graves' disease: case series and review of the literature

Background: Central hyperthyroidism is characterized by elevated free thyroid hormone and unsuppressed stimulating (TSH) this laboratory feature includes TSH-secreting pituitary adenoma (TSHoma) resistance to β (RTHβ). combined with Graves' disease (GD) has been rarely reported. Case Report: We describe three patients TSHoma GD one patient RTHβ adenoma. These showed hormone, while TSH level was normal or elevated, receptor antibodies (TRAb) were positive. After thyrotoxicosis controlled, they all underwent transsphenoidal surgery (TSS). also a an initial presentation of who developed hypothyroidism after anti-hyperthyroidism treatment inappropriately significantly increased. His head magnetic resonance imaging (MRI) revealed Genetic testing confirmed heterozygous mutation in the (THRB) gene c.1148G>A (p.R383H). levothyroxine (L-T4) desiccated tablets treatment, decreased normal. Conclusion: four cases highlight need consider diagnosis central when faced inconsistent function test results, illuminating specific diagnostic therapeutic challenges coexisting primary hyperthyroidism. Finally, we propose clinical management for GD.
